Cyclosporine is a unique immunosuppressive agent isolated from Tolypocladium inflatum Gams (NRRL 8044). It was approved for use in general transplantation, and has been successfully used for kidney, liver, pancreas, heart, heart and lung, and bone marrow transplants. The molecule is extensively metabolized and the ratio of metabolite/parent drug in circulation is often greater than 3:1. Most of the available assays are devoted to measuring the parent drug which is the most active form. In addition, the drug partitions into the red cell and plasma/serum levels are 1/3 those of whole blood.
